First, you must get a copy of the application form. You may fill the application online and have a printer ready to print the completed form DS-156. Note that the DS-156 form cannot be hand written or type written. It must be printed using a laser printer. Once completed and printed, you will need to sign the document. Aside from the form, you will need to be able to present your current and valid passport, two ID photographs, and application fees. Application fees may differ depending on the nationality or country of residence. You should also show evidence of funds for the expenses you will incur when in the United States. You must also show social and economic ties abroad.
Aside from those documents, you must be able to show evidence that the visit is only temporary and that you will leave the country when the authorized stay is over. You must be able to undergo security clearance procedures as well.

You may also fill up the application form DS-156 online and print it. Once you pay the fee, you will get a form DS-157, which you need to fill in too. After that, all you need to do is to schedule an appointment or interview and prepare the requirements mentioned above including the receipt of the application fee you have paid for.
